Use this Questionnaire to apply to adopt a Rescue Kerry, or to introduce yourself to a breeder if you are interested in purchasing a puppy.

The questions are meant to help you determine if a Kerry is the right breed for you, and to guide you in thinking about all the ways a dog will impact your life. The questions address various aspects of dog ownership that should be given serious consideration before you decide to share your life with any dog. Your answers help assess your suitability as a home for a Kerry, and to match a dog to your particular lifestyle and expectations. Please be as specific as possible, and answer all questions as completely and truthfully as you can. It helps us help you!

We suggest a donation to the Foundation of $25 to process the application.

There are two ways to complete the Questionnaire.

1. Print and mail/fax

  • Print the Adoption Questionnaire by choosing either html or pdf format. (To use the nicer pdf format, you will need to download Adobe Acrobat Reader if you don't already have it.)
  • Complete the hard-copy form by hand. (Incomplete forms will not be reviewed.)
  • Snail mail or fax it to the Kerry Rescue Coordinator(s) or Breeder(s) in your area.
  • Please mail your application and tax deductible donation to: Rescue Director, Kerry Blue Terrier Foundation, P. O. Box 1495, Solvang, CA 93464 or fax to 805/686-0809.

2. Complete online

  • Prepare to spend an hour or so completing the form.
  • Complete the form by choosing the online form and entering text in the data fields for all 10 sections. To save your answers in each section, click NEXT, then you can go BACK to review your answers. (Incomplete forms will not be reviewed.)
  • At the bottom of the form, click next to the name of the Kerry Rescue Coordinator(s) or enter the email address of Breeder(s) in your area to whom you want the form emailed. (A copy is automatically emailed to you also.)
  • Click the Submit button at the bottom of the last page.
  • You will be given an opportunity to make a donation with your credit card.
  • Start the online form.

Due to the high volume of applications, our rescue volunteers are uable to respond to all the questionnaires submitted. However, rest assured that each application is very carefully reviewed. You will be contacted when we have a Kerry suitable to your circumstances, this may take days, weeks or months.
